Sheboygan Rotary Club Members,

In the spring of ____, our club will bestow its __th Rotary Good Citizenship Award on an individual who exemplifies the objectives of Rotary. In addition, scholarships will be presented to two outstanding studentsNami when we gather for the annual Good Citizenship/Scholarship Awards Dinner.

The event committee is responsible for the selection of the Rotary Good Citizenship recipient, whereas the scholarship committee selects the student honorees. Please assist the event committee by nominating a qualified individual and returning this form to a committee member by the Nomination Deadline Monday, February 18th, 2019at the close of our noon meeting. We have some nomination records for the past two years. If you wish to resubmit your past candidates name and supporting information, please feel free to do so.
